From October 2018 all Forfar Lists are closed
As from 22 October 2018 all Forfar GP practices' lists will be closed. Any new patients seeking to register with a GP in the area should make contact with Practitioner Services Division by one of the methods listed below in the first instance.

Patients will need to provide their name, address and date of birth for themselves, and any family/household members, as they will be kept together.
Practitioner Services will inform each patient/family which GP practice they are being allocated to so they can approach that nominated GP practice to register. Practitioner Services will also notify the GP practice to let them know which patients to expect.
It should be noted that this arrangement is for unregistered patients only and patients currently registered with a Forfar GP practice who wish to change will only be actioned subject to exceptional circumstances.
