We are a friendly, long established practice with a commitment to providing the highest quality of care and are seeking two similarly minded individuals to work in our Team. Our Team of 4 GPs and 9 practice nurses & HCAs care for a population of 9,400 patients in a semi urban-rural location based in Forfar.

We launched a transformational model of primary care inspired by the Alaskan Nuka system's person-centred model of care in August 2016 and this is now successfully embedded into our multi-disciplinary team. The successful candidates would work closely with the existing Nurse Practitioner Team providing an extensive minor illness service and work alongside a GP to look after their defined patient list. Our aim is to optimise relationships and continuity in order to improve health & wellness outcomes, use of health services and patient & staff experience.

Lead/Advanced Nurse Practitioner Post

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population. As the team leader for the nursing team, the post holder is accountable for nursing service delivery. They will lead and manage all the nursing resources, working closely with the GPs and practice manager to deliver the practice priorities. Clinically, the focus of the role is the delivery of evidence based practice for patients with acute and chronic conditions and management and preventative nursing interventions to all patients. You will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery and procedures and providing leadership and direction for the nursing team.

Nurse Practitioner Post

Working in a small team with a defined patient list, you will be a key team member, responsible for case management, care coordination as well as routine practice nursing duties. Your role will be wide-ranging and will include, for example:

  • Call triage and minor ailments advice
  • Care planning, coordination and follow up
  • Schedule management
  • Managing prescriptions, results and referrals
  • Monitoring chronic disease screening, smears and immunisations
  • Helping patients navigate the system
  • Supporting team development
  • Routine practice nursing duties

Experience in some or all of the above would be preferred, although not essential, as training can be given to secure the right candidate.
