Job Vacancy: Nurse Practitioner
An exciting opportunity has arisen for an experienced and motivated Nurse Practitioner or Practice Nurse to join our friendly and supportive team.
Contract type: Permanant
Hours: Hours: Part Time – Monday & Tuesday (14–16 hours/week). Additional hours available with future flexibility to increase if desired.
Salary: £38,000 (19.48 per hour)– £43,000 (£22.05) per year. Negotiable/Competitive based on experience and qualifications.

We are looking for a dedicated professional who can work autonomously and collaboratively to deliver high-quality, person-centred care.
You will play a key role in managing long-term conditions including:
- Diabetes
- Hypertension
- Heart Failure
- COPD
- Asthma
Key Responsibilities
- Assess, diagnose and formulate treatment plans for acute/chronic illnesses
- Conduct telephone & in-person reviews for patients with chronic conditions
- Manage prescriptions, results and referrals
- Review Medlink questionnaires and interpret test results
- Educate patients on preventative healthcare and lifestyle changes
- Perform core nursing duties including injections and cervical screening
- Collaborate with the wider clinical and administrative team
- Maintain accurate and up-to-date medical records
Essential Requirements
- NMC Registration
- Previous General Practice experience
- Experience in Minor Illness Treatment and Chronic Disease Management
- Registered Non-Medical Prescriber
Desirable Skills & Qualifications
- Experience using Medlink
- Independent Nurse Prescriber qualification (preferred but not essential)
- Strong organisational skills
- Ability to work independently and as part of a multidisciplinary team
