Registering with the Practice

To register as a patient with Academy Medical Centre, you must be resident in our practice boundary area and you must either bring your medical card to the surgery or complete a new patient registration form which is available from NHS Inform's Registering with a GP Practice. Please note: the form on NHS Inform requires a recent version of Adobe Acrobat - if you're told "you need a later version of the PDF viewer" and you don't want to upgrade, please print this version.

Once completed, the form should be brought into the practice - view our location for how to reach us. A copy of the form is also available to collect from our reception desk.

Remember to bring the required identification with your completed form - we require two forms of identification as follows:

  • One form of photographic ID e.g. your passport or photocard driving licence
  • One proof of address e.g. a recent utility bill with your home address shown

Please also complete our patient questionnaire. This helps us get to know you and your preferences.

New patients (over the age of 16) will be offered a health check as part of their new patient registration.

Opening Hours

The practice is open from 8am-6pm, Monday to Friday.

We participate in the public holiday closure timetable as agreed by NHS Tayside for General Practice.
