New Patient Registration

To register as a patient with Academy Medical Centre, you must be resident in our practice boundary area and you must complete a new patient registration form AND our patient questionnaire.

You can download both forms as editable PDFs, complete them, attach to an email, and send them to us:

You may need to download Adobe Acrobat Reader to edit and save your forms.

At the moment (due to Covid) we will accept completed forms without a signature.

Please email the completed forms to the practice at: academymc.tayside@nhs.scot. You do not need to attend the practice to register.

New patient health checks are not routinely taking place due to Covid.
